What Is a Meniere's Disease Specialist?

And why is Meniere's Specialist Care so important?

A Meniere's disease specialist has specific experience treating the condition, understands the latest research, recognizes its complexity, and can offer treatment options beyond the traditional hydrops-based approach.

The science of Meniere's is changing — but the standard of care is still lagging behind. Many patients still spend months or years cycling through traditional treatments based largely on the endolymphatic hydrops model, despite limited evidence supporting many of these approaches. However…

eliminate the distractions

The new paradigm for Meniere's is changing the standard of care.

New research into the origins and mechanisms that drive Meniere's disease is fundamentally changing how we understand and treat the condition.

At NeuroMed Clinic, this emerging science is translated into specialist-directed medical care, creating new options for those who haven't found relief with traditional approaches.

When Should You See a Meniere's Disease Specialist?

Meniere's specialist care can help at every stage of treatment.

1️⃣ At diagnosis: Starting with a Meniere's specialist can help you avoid months of trial-and-error with older approaches such as low-salt diets, diuretics, and betahistine.

2️⃣ When treatment stalls: If you're still having attacks, a specialist can reassess what's driving your symptoms and consider the newest Meniere's disease treatment strategies.

3️⃣ When symptoms overlap: Meniere's can overlap with vestibular migraine, PPPD, and vertigo with tinnitus. A specialist can identify these overlaps and treat the full picture.

4️⃣ Before escalating treatment: Before injections, ablative procedures, or surgery, an up-to-date Meniere's specialist can help determine if less invasive options are still available.

What's New in Meniere's Disease Specialist Care?

Effective new treatments are now available… and many still don't know about them.

The biggest change in Meniere's care is a new understanding of what's really driving the disease. Research increasingly points to migraine-related neurological and neurovascular mechanisms, creating treatment options that go beyond the outdated fluid/hydrops model.

And the results are promising. In a UC Irvine study led by Dr. Djalilian's team, 92% of patients with definite Meniere's disease who had already failed diuretic therapy reported improved quality of life after migraine prophylaxis treatment. Improvements were seen across physical, emotional, and social measures.

92% improved quality of life

23 of 25 patients with definite Meniere's disease improved after migraine prophylaxis treatment, with at least 18 months of follow-up.

NeuroMed builds on this research. Our approach combines medical interventions with neurological and vestibular rehabilitation to address the full spectrum of Meniere's disease symptoms.

Meniere's Disease Posts

The Best Meniere's Disease Diet: More Than Low Sodium The best Meniere's disease diet isn't built around sodium restriction — controlled evidence hasn't shown low-salt diets prevent attacks, and endolymphatic hydrops doesn't reliably track vertigo severity. The optimal Meniere's diet instead works by stabilizing migraine biology: managing trigger foods, keeping

Betahistine for Meniere's Disease: Does it Work? Betahistine for Meniere's disease does not appear to work: the strongest placebo-controlled trial found no reduction in vertigo attacks, and systematic reviews rate the evidence as insufficient or very low certainty. Betahistine targets endolymphatic hydrops, but hydrops appears to be a downstream consequence

Rethinking Endolymphatic Hydrops Endolymphatic hydrops is a swelling of endolymphatic fluid compartment of the inner ear, and it's long been treated as the cause of Meniere's disease. But newer research shows hydrops doesn't reliably explain the vertigo, and treating it doesn't reliably stop it. Here's what the evidence actually says.

Diuretics for Meniere's Disease: A Closer Look Diuretics for Meniere’s disease are routinely prescribed, but they have not been reliably shown to prevent attacks. The latest Cochrane review rated the evidence low or very-low certainty. One reason may be that diuretics target endolymphatic hydrops, while newer evidence suggests hydrops may

Meniere's Disease Low Salt Diet: Does It Work? The Meniere's disease low-salt diet has “unknown effectiveness.” Two large systematic reviews found insufficient evidence that sodium restriction reduces attacks, despite its continued use as a first-line treatment. The recommendation is rooted in the endolymphatic hydrops theory, but newer research increasingly points

Meniere’s Disease Treatment: What Actually Works Meniere's disease treatment has traditionally included lifestyle changes, medications, injections, and surgery. Diuretics and low-sodium diet have long been the starting point, but the evidence is weak, many patients don't respond, and it often just delays treatment. Newer approaches targeting the brain processes driving

Meniere's Disease Specialist — FAQs

Meniere's disease is commonly treated by an ENT, but complex or persistent cases may benefit from Meniere's specialist care. A specialist approach should incorporate current Meniere's research, recognize overlapping vestibular conditions, and offer treatment options beyond traditional first-line care.

An otologist or neurotologist is the type of ENT that most commonly specializes in Meniere's disease and other complex inner-ear disorders. These physicians have advanced expertise in hearing and balance disorders.

At NeuroMed, all of our clinical providers receive specialized training from Dr. Hamid Djalilian, an otologist, neurotologist, and leading researcher in Meniere's disease treatment. This allows patients to receive specialist-directed care throughout the program.

You no longer necessarily need a Meniere's specialist physically near you to receive specialized care. NeuroMed provides specialist-directed Meniere's treatment through telehealth, with local testing or in-person services coordinated when necessary.

The best Meniere's disease specialist for you should understand the latest research and be equipped to treat the full complexity of your condition. Look for expertise in Meniere's disease, vestibular disorders, migraine overlap, hearing symptoms, persistent dizziness, and both medical treatment and rehabilitation.

Consider Meniere's specialist care early, particularly if standard treatment isn't working or more invasive treatment is being considered. Specialist care can also be valuable when the diagnosis is unclear or symptoms overlap with vestibular migraine, PPPD, chronic dizziness, or complex tinnitus.

Failure of a low-salt diet, diuretics, or betahistine does not necessarily mean you've exhausted your Meniere's treatment options. Newer approaches informed by migraine and neurological research may provide additional options before progressing to ablative procedures or surgery.

Across the 20-week program, you'll have 15+ telehealth visits with clinicians trained in Dr. Djalilian's Meniere's treatment protocols. At each visit, your care team tracks your attacks, hearing symptoms, dizziness, balance, and other symptoms, then adjusts treatment based on how your particular case is responding.

Between visits, treatment continues on multiple fronts: medical therapy targets the neurological mechanisms contributing to Meniere's attacks, vestibular rehabilitation works to restore balance and motion tolerance, neurocognitive and autonomic rehabilitation address persistent sensitivity and nervous-system dysfunction, while sleep, nutrition, and lifestyle interventions help reduce factors that can trigger or sustain symptoms.

This a customized Meniere's protocol. What changes over the 20 weeks is the emphasis placed on each component, based on your symptoms, response to treatment, and progress over time.

No treatment provides 100% recovery for everyone, and we won't promise otherwise. What we can say is that NeuroMed's protocols help over 90% of patients experience meaningful functional improvement and increased quality of life.

This isn't a generic estimate. It's grounded in Dr. Djalilian's own published clinical research on migraine-based treatment for vestibular migraine and Meniere's disease specifically.

The NeuroMed Meniere's Disease program was designed from the ground up as a telehealth-based dizziness and vertigo rehabilitation program, not adapted from an in-person clinic model. That distinction matters.

Successful Meniere's disease treatment depends heavily on:

  • Longitudinal symptom tracking,
  • Ongoing treatment adjustments,
  • Regular and consistent medical coaching,
  • Nervous system regulation, and
  • Structured rehabilitation delivered over time.

None of those things require you to sit in a physical office. What they require is continuity, and telehealth is often better suited to providing continuity than traditional periodic office visits.

Practically, this means:

  • No travel or waiting rooms
  • Medical appointments conducted through secure video visits
  • Supplements delivered directly to your home
  • Prescriptions sent electronically to your local pharmacy
  • Educational modules, coaching, and vestibular therapy tools accessible remotely

For your Meniere's, care that fits into your life rather than constantly interrupting it is not a compromise. In many ways, it is an advantage.
